    You can also chose to take $10 instead of a magazine subscription. To redeem for the cash reward instead, go to https://www.bperx.online/ and click the link to go to the forms page.

    You have to print the page, fill it out and mail it. You just have to fill out your name, address, email and phone number plus the gift voucher number.
