  Hi there! Be sure to click through the link above and fill out your information, including your birth date. When clicking on the “birth date” portion of the form I am able to click on a little black arrow to find a calendar pop up where I can select my birth date. Hope that helps!

  Hmm, I still find the box for your Birth Date listed in the form, through the link above. Maybe try again in different browser OR switch to desktop view if using a mobile device. Hope this helps!

   I signed up over a year ago and I still have yet to receive a free bang bang shrimp for my Birthday. My mom gets it for hers but I have never gotten it.
